Requirements for the Metadata Sharing Server are being gathered. It is designed to replace this page with a better tailored approach. If you have any suggestions, please visit the project page and contact us.
This form bank is an exchange area between OpenMRS implementers to share electronic forms. Please consider exporting your forms with the Metadata Sharing Module and sharing them here. Contributors should attach the OpenMRS form to this page, along with an optional original paper form. It is recommended that metadata sharing module is used to create a zip file which contains the form along with all dependencies (concepts, datatypes, classes, etc). The form could be htmlform, Xforms or an Infopath form. To use a form from the form bank, you will need to download and install the Metadata Sharing Module. You can find instructions for importing metadata (the XML files on this page) in the User's Guide for Metadata Sharing.
Forms can now be exported between various versions of OpenMRS. See OpenMRS ticket [META-44] and [META-168]

The recent release of Metadata Sharing 1.0 supports 1.6/1.7/1.8 compatibility, and forms exported with it should be compatible with future MDS versions too, so it would be awesome if people can re-do these form exports using MDS 1.0!

Can any of the above forum work with the platform 1.10 or is there is any modification can be done to make them work?

You should try any of the htmlforms which were created on OpenMRS 1.9. I can't think of incompatibilities between the forms and concepts on 1.9 and 1.10.

Unfortunately I don't have any of these forms on 1.10, but James Arbaugh is usually on the leading edge. James?

Thanks Ellen Ball. Those forms were from HAS, and they are still running OpenMRS 1.9.x. Sorry osama shoeib, but it looks like you're out of luck with OpenMRS 1.10 forms at the moment.
As a work-around, you could import them into the appropriate version (1.9 for example), and then upgrade the implementation to 1.10, and export them again with Metadata Sharing Module in that format. You could use the legacy standalone version of OpenMRS for this. Just replace the old WAR file with the 1.10 war file after you're imported the form.

osama shoeib, I have added some OpenMRS 1.10 forms to the Form Bank. The Vaccination form uses a tag that requires the latest version of HTML Form Entry 2.5, which has not yet been released.
